Medication Policy  Immunization Changes Meningitis Information

 Kindergarten students need:

1.       Four or more doses of diphtheria , pertussis and tetanus vaccine, with at least one dose on or after age four.

2.       Four or more doses of poliovirus vaccine, with at least one dose on or after age four.

3.       Two doses of measles, mumps and rubella vaccine (MMR).  Minimum age for the first dose is 12 months.

4.       Two doses of varicella vaccine (chicken pox).  Minimum age for the first dose is 12 months.  History of disease is acceptable with a parent/guardian signature.
